1. Make sure you have turned ON the ‘Advanced Options’ 
2. Select ‘Create New Form’ 
   
   
   
   
3. Click on the downside arrow located at the right side of the ‘Form Questions’ option 
   
   
   
4. You can rearrange the order of the questions.
5. You can turn them ON or OFF by clicking on the switch next to the question.
